Veterinary Consent Form

The completed consent form must be returned to me prior to your dog's first treatment, please allow at least 7 working days. Feel free to message me if you are having issues getting the consent form signed.

Vet consent form will need to be renewed if the dog has a new condition or if they haven’t been seen for six months. Animals attending arthritis clinics do not require veterinary consent unless combining with a veterinary physiotherapy appointment.
